Why choose a Nurse Practitioner?
A nurse practitioner is a registered nurse who has completed advanced or graduate level education to perform certain duties traditionally performed by a physician. In the state of Wisconsin, a nurse practitioner can assess patient’s, order and interpret diagnostic tests, diagnosis conditions, and treat those conditions in collaboration with a physician.
Payment Policy
Cash Pay. We are a cash only practice, meaning Route One Medical Spa does not accept private insurance, Medicare, or Medicaid. Payment may be made by credit card, cash, or check. You agree to pay any bank fees for returned checks. You understand that you are wholly responsible and liable for payment of all charges and fees assessed for services rendered. Please contact us if you have any questions about our payment policies.
Good Faith Estimate. You are entitled to receive a Good Faith Estimate of the expected charges for your services. Practice will provide you with a Good Faith Estimate before your scheduled services or upon request.
Cancellation Policies
Cancellations. We have reserved your appointment time specifically for you. Please be on time for your appointment. If you are more than 10 minutes late to your appointment, we reserve the right to reschedule. If you need to cancel or reschedule your appointment, please contact me during business hours and at least 24 hours prior to your scheduled appointment to avoid paying a late cancellation fee of $50. For Monday appointments, you must cancel by 12:00 p.m. on the previous Friday so I can offer that time to another patient. If you do not show up for your appointment, we will charge you a no-show fee of $50.
